Social Services
The role of the Social Services Department, as members of the care team, is to provide patients and their families with counseling and referral services to help cope with the problems that often accompany an illness or hinder recovery and rehabilitation. As key members of your health care team, they are devoted to helping people function the best they can in their environment.

Social Services can assist individuals with the emotional, physical, and financial necessities of daily life through casework, advocacy, and appropriate referrals. Our staff can help ease the worries and concerns that can prevent patients from obtaining the maximum benefit from their medical care and can offer assistance and guidance to meet a variety of emotional and social needs such as:
Adjustment to illness, disability, or loss
Advance Directives, information, and guidance
Behavioral health needs
Child/Elderly protective concerns
Domestic violence
Discharge planning for patients needing rehabilitation or long-term care
Family/Relationship issues
Financial/Legal problems
Referrals for basic needs such as housing, transportation, and medications
Substance abuse/chemical dependency
Advance Directives
Advance directives are legal documents that state a patient's preference in treatment and resuscitation - if the patient is unable to speak for himself or herself. There are two types of advance directives; you are encouraged to fill out these documents provided for you here. Simply click on the highlighted link.
Living Will
- This document states a patient's wishes in the withholding or withdrawing of life support, if the patient suffers from an incurable and terminal condition.
Durable Power of Attorney for Healthcare
- This document designates another person to make healthcare decisions if the patient is no longer able to make them. This designated person also has the power to make the final decision about cessation of treatment.
