CPPC Recieves PCMH recognition

Varnville, SC January 26, 2023 - Coastal Plains Primary Care at Hampton Regional Medical Center in Affiliation with MUSC Health has earned the Patient-Centered Medical Home Recognition by the National Committee for Quality Assurance.

NCQA's PCMH program is the most widely-adopted evaluation program in the country, with approximately 13,000 practices recognized. The patient-centered medical home model of care has been research-proven to enhance both patient and staff satisfaction while reducing healthcare costs.

We are thrilled to be acknowledged by NCQA's PCMH Recognition program," said Dr. Malcom Horry, medical director of Coastal Plains Primary Care. "Not only will it enable us to continue to provide affordable quality healthcare to our patients, but it will also help us improve the patient experience and develop more meaningful relationships with those that we serve in our communities."

"NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients," said NCQA President Margaret E. O'Kane. "Recognition shows that Coastal Plains Primary Care has the tools, systems and resources to provide its patients with the right care, at the right time."

The NCQA PCMH program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts. The NCQA PCMH standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
